Hmm, I'm a little confused by the question. If you're asking about the regular OpenSim terrain then it isn't mesh-based. E.g., you can't create a cave using the terrain tools. This type of terrain doesn't affect the speed of the world, no matter how many mountains or jagged hills you create. But if ...

The speed of the simulator is not affected by the number of assets, but by the number of prims, scripts, physics etc. So assets in boxes don't affect the runtime speed.
