The easiest way is to put the hypergrid address into your "Find" on the Map tool, just like if you were on SL and searching for a sim using the map. Most the HG-enabled grids have their address on their webpage.
You can get a free grid teleporter at Fleepgrid, though I haven't played with it, so I am not sure how to set it up. Address: fleepgrid.com:8002

Hi Roxy,
Please note that you don't need to log out of your viewer in order to hypergrid teleport between grids. You can be inworld in Kitely with your Kitely avatar, enter the address of a region on another hypergrid-enabled grid and teleport with your Kitely avatar to that grid.

Avination is not open to the hypergates. Melanie had been saying for some time that she would eventually enable it there under certain circumstances, but I have not, personally, seen any updates on that for quite some time now...but tbh, I don't find that grid interesting enough to watch for news there, but really as it is still closed.
OpenSim is not down, just the OSGrid part of it is. Since OSG is the largest by region count, and the main testing grid for OS, this is a problem, but they recently reported things are still being worked on. http://news.osgrid.org/2014/09/12/recovery-details/
Metropolis uses core OpenSim and is a great destination grid on the HG. Goto the welcome region via hypergrid.org:8002 and make sure and pick up the sightseeing folder over by Bertha, the Metro mascot...lol
Quite a few ppl have, mostly temporarily, I would imagine, moved from OSG to Metro and Craft-world [another HG enabled Grid] and I would like to think to Kitely also.
Metropolis is one of the grids who have been supporters of Kitely and it's MP.

and fyi, you can take a copy at the landing place in Sanctuary, of his teleporter which goes back to him, but can be easily modified for any other places if desired...i think i recall he had instructions in it...

Please note that, for security reasons, the script functions that invoke a forced teleport can only be used inside your own virtual world. Without this measure grieffers could have easily harassed people by force teleporting them without the victims being able to do anything about it (if someone does it in his or her own world then people can simply avoid visiting that world in the first place).
