Parental/Guardian Permission Wording

Hi Everyone,
I have created a sim with the aim of personal development. It will most likely attract adults, but I am open to 16-18 year olds attending as well. No younger than 16 though, because they may be unsupervised outside of any formal activity. I haver noted very carefully Kitely's Terms of Service in this.
I am in the process of creating a community code of conduct and am wondering if you would be open to sharing with me a description of parental/guardian permission wording and processes you may use.
