Am I right in thinking that Internet Radio is free if you use their own stations, but that you pay if you want to start your own.

That depends on the country you're in and its own laws relating to streaming music. The way OpenSim works is that the viewer acceses any media URLs it encounters directly so that the music stream data is never handled by the grid or region that is using that media URL. In other words, the grid only provides the viewer with the address of the radio channel that is hosted by third parties on different servers and your viewer then acceses those channels directly.
