Yes, you can go to the My Worlds page and press the New World button to create your own world(s). Select the Premium World option as the world type if you want to take advantage of the 5 free regions the Premium Account entitles you in Premium Worlds. I find it easiest to shop on the Kitely Market, to be honest. There are some good inworld shops, too, but it's quicker to find and buy specific things via the Market website.
